Referrals

A calendar will be created, maintained and regularly shared to guide the scheduling of My LIFE meetings so that all involved parties will be aware of the youth participants and timelines. The My LIFE Coordinator will be responsible for ensuring that all meetings that form part of the My LIFE Transition Program are in place for each youth participant and that all involved parties are in attendance. Social Workers will be responsible for providing the ILP Coordinator with relevant context and background of the youth participant.

Pre-Work Meetings

The ILP Coordinator will schedule and facilitate the Pre-Work meetings with the participant and youth advocate. The Pre-Work meetings serve to complete a comprehensive assessment, identify participant goals and strengths, determine invitees and provide the participant with an overview of the My LIFE meeting format, purpose and outcomes. The youth advocate will be responsible for maintaining relevant documentation during the pre-work process.

Strategy Meetings

Upon completion of pre-work the ILP Coordinator will schedule a Strategy meeting with the youth advocate and My LIFE Coordinator to prepare for the 1st My LIFE meeting. The ILP Coordinator will provide information regarding the participants’ goals and potential barriers, including any relevant information provided by the Social Worker. At the Strategy meeting the list of invitees will be provided to the My LIFE Coordinator. The My LIFE Coordinator will determine location and invite attendees at least 2 weeks in advance of the scheduled My LIFE meeting.

1st My LIFE meeting

The youth advocate and My LIFE Coordinator will be responsible for preparing the documentation and facilitation tools prior to the 1st meeting. The My LIFE Coordinator will facilitate all My LIFE meetings and ensure that the participants are clear about purpose, outcomes and agenda items (see attached Agenda.) During the meeting, the ILP Coordinator will be responsible for completing the TILP and handing it off to Human Services Department (HSD) to input into CWS/CMS. The youth advocate will be responsible for taking notes during the meeting. Additionally, any follow-up meetings will be scheduled during the 1st My LIFE meeting.

After each My LIFE meeting the youth advocate and My LIFE Coordinator will debrief with the participant to make any changes to the meeting format that better support their needs. The youth advocates will be responsible for sending out the notes within one week of the meeting. The My LIFE Coordinator will be responsible for ensuring that any follow-up is conducted and the timeline for Pre-Work and Strategy meetings are set for any additional My LIFE meetings.

Follow-up My LIFE Meeting

The Follow-up My LIFE meetings serve to support the youth participant in adjusting and refining the goals and plan set forth in the 1st My LIFE meeting. The My LIFE Coordinator will facilitate the meeting reviewing the purpose, ground rules and each individual’s role at the beginning of the meeting. The LIFE Plan from the 1st My LIFE meeting will be distributed and the participant’s strengths will be discussed and revised as appropriate. The participant’s completed goals and other milestones will be acknowledged and celebrated.
Then each goal of the My LIFE Plan will be reviewed and updated as needed including the development of new action items with corresponding leads and deadlines. If appropriate the TILP will be updated by the ILP Coordinator and additional meetings will be scheduled. The youth advocate will collect meeting notes and transfer to the LIFE Plan document within two days. All relevant parties will be emailed or mailed a copy of the revised My LIFE Plan. HSD will be responsible for inputting the revised My LIFE Plan into CWS/CMS.

Collaboration

A quarterly collaboration meeting will be implemented to ensure that all agencies participating can give feedback regarding program refinements and coordinate the participant calendar.
